Bookstore Wholesale Order Form

Thank you for your interest in my books. This form is for retailers who wish to purchase signed physical copies directly form me. Books will be sold at 40% wholesale pricing with a shipping charge of $5.22 (standard USPS book rate) applied for one book and an additional $1 for each additional book.
